5 hours per week (Saturday and Sunday) and is eligible for benefits.
JOB DUTIES:
Conduct home visits and patient assessments Admit patients Incorporate appropriate assessment and intervention skills in all interactions and delivery of nursing care Communicate with clients, team members, and management in an accurate and timely manner Integrates the influence of the human experience of health and healing, illness or dying and death Adapts care management for diverse populations and environments Advocates for needs of clients Uses effective time management strategies including triage and prioritization Incorporates teaching in delivery of care with a focus on health promotion Completing paperwork and documentation in a timely manner Conduct comprehensive initial and ongoing in-home psychosocial assessments of patients' circumstances, problems and needs related to their illnesses need for care and response to treatment.
